Projects

Our mission is anchored in project-based initiatives that use empowerment for better health outcomes.

Somalian Community Project

The Somalian Community Project is co-designed with Somali residents in Brisbane to address health and social challenges for people with disabilities. It aims to empower the community to build confidence in self-advocacy and improve access to mainstream health services.

Project goals include:

  • Develop resources that adapt the health and social framework of disability to align with Somali cultural values and experiences.

  • Equip the community with the knowledge to self-advocate effectively within the healthcare system.

  • Create a co-design methodology that can be tailored to meet the needs of other communities facing similar challenges.

By focusing on culturally relevant approaches, the project strives to create resources that connect with the community.

Current Status: This project commenced in August 2024, and completed in June 2025. Xstitch Health is proud to have competed this project without any government, grant or philanthropic funding. This demonstrates our commitment to self-sustaining innovation in Health.

AdvoKey Program

The AdvoKey program was co-designed with residents at an accommodation service for older men experiencing homelessness.  It focuses on empowering them to build healthier, more independent lives. We are currently approaching sponsors for this program.

The program has four key goals:

  • Equip residents with an understanding of their health conditions and their long-term impact.

  • Help residents learn how to manage their health conditions effectively, including medication routines and recognising symptoms.

  • Teach residents how to communicate effectively with General Practitioners to get the care and support they need.

  • Provide practical resources for meal preparation and household cleaning to improve daily living and self-sufficiency.

These goals were developed through in-depth conversations with residents and supported by research insights. By focusing on long-term health and lifestyle improvements rather than short-term healthcare access, AdvoKey aims to empower residents to thrive when transitioning to permanent housing.

AdvoKey is led by Xstitch Health and ESG360, with strong collaboration from community partners:

We are also very grateful for the support of Cr Danita Parry, Brisbane City Council local member for Marchant Ward.
